D512 not working anymore

My D512 box stopped working half way through a gig last night, the power light is always on but PC link does not stay lit. If I restart the computer I plug it in, it works (PC link blinking) for around 15 sec then stops, then computer comes up with 'USB Device Not Recognized', and usually crashes every USB device.
I've tried a system restore, reinstalling the drivers, reinstalled the software, using both the demo show and my light show, and installed it on two different computers (with the same results).
I believe it's got to be a faulty box (I've only had it a few weeks), how do I go about getting a replacement?
